In April 2024 at Aurora Media, Drew Cain offers Willow Corinthos a volunteer spot in the charity The New Tomorrow Institute which deals with bone marrow donors, stem cells, and organ transplantation. As Willow recently recovered from leukemia, Drew thought it would be perfect for Willow to be a spokesperson. Willow says she will think about it.
On May 7, despite Drew's worries that Willow feel obligated, she accepts his offer to be spokesperson for the charity. Nina hears this and is proud of Willow. Drew wants Crimson Magazine to do a story on the charity and Willow's involvement.
On May 13, Willow shoots a rehearsal PSA for the charity with Drew filming and her mother Nina Reeves observing.
At the annual Quartermaine 4th of July party, with half the town tuned into the TV and radio, Drew steps up to announce his candidacy for Congressman and his decision to adopt the family name Quartermaine, leaving behind the name Cain. On the same day, Drew and Willow's affair begins with a kiss.
On August 9, Willow runs into Drew in the Metro Court Hotel elevator, after picking up some outfits from Crimson for her upcoming PSAs. Drew has good news for her from the Institute, but is interrupted by the conveyance jerking to a halt.
A call with Olivia Quartermaine tells them the elevator company is on the way. Finding it warm, Drew takes his jacket off, loosens his tie, and rolls up his sleeves. Willow is visibly uncomfortable and asks him about the good news. The Institute loves her and her work so far - the director wants to fly them out to L.A. for a big fundraiser. A villa has been rented for them, which is a "magical spot." Willow, concerned about their growing attraction, blurts out, "I can't!"
She covers by stating that her son and Drew's great-nephew Wiley Corinthos is about to start a new school, and they have a family trip before then. She worries if regular trips are to be expected, maybe this isn't the role for her. Drew tells her it's no problem, that the Institute understands she has two children. He then asks if he is the reason she doesn't want to go and if so maybe they should talk about it.
Willow reminds him they agreed to leave the Fourth of July and their kiss behind them, and there's no need for further discussion. They both tell one another how much they appreciate each other.
The elevator begins moving again. Drew thinks Willow should go to the benefit and he'll stay home. She insists she doesn't want him to miss out. He admits that she's one of his favorite people and only wants her to be happy. Suddenly the doors open, and when they see her mother and his secret fling, Nina, standing there - Willow rushes out.
On August 14, having regained her nursing position at the hospital, Willow announces she's leaving her position at the Institute. She claims that she finds more fulfillment in nursing and the Institute position required too much travel. Privately, Drew asks Willow if it's because of their feelings towards each other. She admits she can't stop thinking about him and needs the distance. She agrees to continue the PSAs until a replacement spokesperson is found.
Drew admitted the first kiss to Michael Corinthos the day prior, leading to Michael walking out, angrily getting drunk at a bar, and ending up in bed with Sasha Gilmore, his ex-fiance and best friend of Willow, who was drunkenly dealing with her own heartbreak.
On Halloween night (shown on November 5), following the announcement of Sam McCall's tragic death, the Quartermaine household joins the rest of the town in mourning - especially her ex-husband Drew, their daughter Scout, and Michael whom she'd always been close to. Willow finds Drew in the play room, and in offering him comfort, they end up having sex.
At the Christmas party on December 17, Drew and Willow's affair was outed when Tracy accidentally played the nanny cam footage of their one night stand. Nina rushed Willow into taking Wiley and Amelia and coming to stay at her penthouse. Drew himself went to stay at the Metro Court's rival, The Cosmopolitan Hotel.
Willow was happy to accept an invite from Drew to accompany him to his congressional swearing-in ceremony. She, Wiley, and Scout all go to D.C. with him - and a photo of them looking like a happy couple is seen on news sources.
On Valentine's Day (aired on February 19, 2025), he shows Willow his new house, sharing that Lucy Coe found it and Maxie Jones decorated it. He then invites Willow and her two children to move in with him.
Willow loves the house, but turns down the offer to move in - stating they've bounced back and forth between different residences in the last few weeks, and need stability.
On February 26, 2025, Willow reaffirms her commitment to Drew and their relationship. She wants to make a statement to show the world how committed they are and that their love is real. Therefore, she decides that she and her kids will move in with Drew after all.
On April 10, 2025, Willow, Wiley, and Amelia, officially move in with Drew. However, several family members such as Tracy Quartermaine, Jason Morgan, Carly Spencer, and Nina are against this and set out to stop the move.
On April 14, worried that their children could be taken from them, Drew and Willow sign documents giving each other custody of their children in case of emergency.
On June 18, after Willow loses custody of Wiley and Amelia, Drew proposes to her. He felt they were close to marriage before the custody battle and he wanted to make the proposal more romantic, but Martin Grey recommended marriage as a legal strategy to strengthen their position for future custody arrangements. Drew thinks if they are married, it would look more favorable to the courts when they try to revisit the issue of custody. Willow is unsure and says she needs time to think about his proposal before accepting.
