On March 8, 2022, it was revealed that Ryan has been faking his locked-in syndrome. After Ryan was taken back to his cell, Esme steals an access card and secretly visits him. It was revealed that Esme was working to break up Nikolas and Ava under Ryan's orders and it was revealed that Ryan is Esme's father. Ryan implies that he wants Esme to seduce Nikolas. She starts heavily flirting with Nikolas and briefly stayed above Kelly's before she ended up living at Wyndemere with Spencer although Ava wanted Esme out.
On March 29, Sonny has Esme kidnapped. Nikolas and Victor come to her rescue and Esme throws herself into Nikolas' arms while crying over-dramatically.
On April 13, 2022, Esme told Nikolas that her parents were dead and her adoptive parents suddenly died. She was taken care of by her nanny Maggie until she went to boarding school and never heard from Maggie again. Later Esme pulls out a letter written to her at boarding school from her nanny Maggie Fitzgerald who lived in London. Maggie warned Esme about going to Port Charles as Esme's father was nearby proving that Esme lied and she did hear from Maggie again.
Ava is furious that Esme is still there and cancels her vow renewal with Nikolas. He argues that Esme is in danger from Sonny. Ava admits she told Sonny to take Esme. She also has Nikolas sleep in a different room until Esme is gone. Nikolas later reveals that he is onto Esme but she continues to stay close and be clingy with him.
After a couple months of manipulation, flirting, and stroking his ego, Nikolas gives into the temptation Esme offers, and they sleep together.
Nikolas eventually comes clean with Ava and Ava confronts Esme at Wyndemere. When Esme tries to keep a revealing letter from Ava's hands a struggle ensues and Esme falls over the parapet, she is later presumed dead. In October, Esme is revealed to be alive at the same time that a serial killer stalks the town with a hook. She breaks into the castle, in an attempt to steal money, and Nikolas catches her. He's shocked to discover she's pregnant - with his child! She agrees to a paternity test, and blackmails Nikolas into not turning her in to the police, hiring her the best lawyer Cassadine money can buy, and letting her stay at the castle. Nikolas turns the tables on her by locking her into a large en suite in a wing of the castle no one uses, only for storage. She been held at Spoon Island there secretly as a hostage by Nikolas until she gives birth, with Elizabeth Baldwin in on the plan, and taking care of her prenatal needs as her private nurse. Nikolas plans to send Esme to Cassadine Island after she gives birth. On New Years Eve 2022, Esme set fire to the storage room using a match in order to escape Nikolas, then she intentionally falls off the parapet into the ocean. She survives, but has amnesia, and was taken into custody at Spring Ridge Prison. On February 6, 2023, Nikolas was presumed dead after being struck in the head by his ex-wife, Ava Jerome during an argument and was placed at the Wyndemere Stables with the help of Austin Gatlin-Holt. On February 15, 2023, the baby was delivered with the help of his paternal grandmother Mayor Laura Collins and maternal great uncle Dr. Kevin Collins, and paramedics, while Nikolas was presumed dead prior to the baby birth. Nikolas was later found by Austin's cousin Mason Gatlin and was held in an unknown location for an unknown person, while he was on life support.
