On August 20, 2018, Valentin Cassadine visits Madeline Reeves in Pentonville Penitentiary. He questions her about the story that her daughter Nina lost her baby while in a coma. Valentin studied Nina’s medical records and noticed an eight month window where she was not at Crichton-Clark Clinic. He asks Madeline if Nina had a son. After he doesn’t get the reaction he wants, he asks if it was a daughter and realizes it was. Madeline finally admits that Valentin figured it out.
She explains that she got the dosage wrong when she drugged Nina to kill her baby back in the 90s. Nina went under but the baby held on. She couldn’t try to drug Nina again or suspicion would fall on her instead of Nina’s ex-husband Silas Clay.
Madeline said she hired a discreet nurse (later revealed to be Phyllis) who looked after Nina until she delivered. Then Nina was transferred back to Crichton-Clark.
Madeline reveals that she put the baby up for adoption anonymously. That way they couldn’t identify each other in court. She says she still has a way to track down Nina’s daughter but Valentin argues with her and says that he hopes she rots in jail before angrily leaving.
On August 28, Nina received a call from Pentonville informing her that Madeline was dead.
On September 7, Nina received a package from her late mother's lawyers and it contained a locket she loved as a little girl. But half of it was missing (Madeline also wrote in a letter that the locket could mend a broken heart) so Nina deduced that her child must be alive and they have the other half.
The same day, Jordan and Curtis Ashford visit Carter Buckley, the lawyer who brokered the illegal adoption of Nina’s child. He didn’t have much information except to say that Madeline had the child sent away. Curtis concluded that Madeline must have hired a private nurse (Phyllis) and a doctor who delivered Nina’s baby.
On September 14, Nina asks a jeweler named Ahmad about the half heart necklace. He confirms that it looks like it was divided from a full heart pendant but doesn’t come up with anything else.
On October 1, a DNA test confirms that a woman named Sasha Gilmore is Nina’s daughter with Silas. However, on November 2, it is revealed that Valentin hired her to pretend to be Nina's daughter.
On April 1, 2019, two more DNA tests confirmed that Sasha is not Nina’s daughter with Silas.
On April 9, 2020, it's revealed that Nelle Benson-Jerome is Nina’s daughter with Silas and Nina also has a grandson named Wiley Quartermaine-Corinthos. Nelle is seen with a half heart necklace in her jewelry box. She is also shown in a flashback as a little girl wearing the necklace when she lived with her adoptive father Frank Benson.
On August 12, Nina tells her boyfriend Jax about the necklace and he calls someone who may be able to find out more about it. Nina and Jax later meet with Jax's jeweler, Evelyn, who tells Nina that she might be able to find the original craftsman.
In August, Nelle returns to the Corinthos Cabin with Wiley to hide out. Michael finds her at the cabin and rescues his son who was kidnapped by Nelle. Nelle runs away and Carly confronts her but Nelle ends up falling off a cliff. Her body is not found but her half heart necklace is on the ground. A police officer looks around the area that Nelle fell and doesn’t see the necklace. Later Nelle’s body is found on the Pennsylvania side of the river and it is concluded that she drowned.
Evelyn returns on September 1 and informs Nina that the necklace’s clasp has an interwoven set of initials on it. She traced the hallmark to an artisan in Manhattan.
On September 10, Nina and Jax go to Manhattan to meet the jeweler. Nina says he hasn’t returned any of her calls. She wants to back out since the jeweler is retired and she doesn’t want bad news but Jax convinces her to knock on the door.
Jax asks if the man who answers is a jeweler named Abraham Dobkin and the man says he is. Nina shows the necklace and says it was divided. Abraham mentions that he usually makes full hearts but that request sounds very familiar.
He remembers the lady who asked for the necklace to be divided. Nina shows a picture of Madeline and asks if it is her. Abraham says he has never seen her before. He does keep receipts and finds one from September 1991 for a woman named Phyllis Caulfield. Jax asks Nina if she recognizes the name and she says she does.
Nina later reveals that she remembers Phyllis as the nurse who took care of her after she came out of her coma at Crichton-Clark. Nina assumes Phyllis must have helped Nina give birth. Nina tells Jax they need to find Phyllis but she puts the search on hold for now.
In September 2020, Ava convinces Sonny to let her have Avery Jerome-Corinthos but she has to agree to stay at the cabin. Once there, Ava hits an intruder on the head with a fire poker and he turns out to be her husband Nikolas. They argue briefly before making love in the cabin.
In the meantime, Avery wanders in the woods and finds the necklace which she takes with her. After Avery is found, she later shows the necklace to Sonny.
On October 19, Valentin offers to help Nina search for Phyllis. Nina refuses his request.
On October 23, Curtis accepts Nina’s previous offer to locate Phyllis.
On October 27, Curtis tells Nina that he narrowed his search down to six nurses who worked in the 90s with the name Phyllis Caulfield.
