Between October 2007 and March 2008, Port Charles was terrorized by a serial killer known as the Text Message Killer, who was eventually revealed to be Diego Alcazar.

Emily Quartermaine was one of the Text Message Killer's victims.
On September 10, 2007, Leticia Juarez, who served as the nanny for Michael and Morgan Corinthos, was discovered deceased in the residence of their mother, Carly Spencer. Initially, the primary suspect was Anthony Zacchara; however, the focus of suspicion later shifted to his son, Johnny Zacchara, after Carly began receiving text messages from the murderer. Carly consented to meet the assailant at the Black and White Ball, an event organized at Wyndemere Castle by Nikolas Cassadine and his fiancรฉe, Emily Quartermaine. During the occasion, Carly was assaulted, but Emily ultimately became the subsequent victim of the Text Message Killer that night. It was initially thought that the killer was Anthony Zacchara, who was paralyzed at the Black and White Ball and subsequently arrested.

Georgie Jones grew suspicious that her sister's boyfriend, Cooper Barrett, might be the murderer. To investigate, Georgie enlisted Damian Spinelli to look into Coop's background. In his findings, he discovered that Coop was a military veteran and that a woman had been raped and strangled to death in the Iraqi town where he had served, making him a suspect. The women murdered by the Text Message Killer had also been strangled, which raised further concerns about Coop's potential guilt.
A few weeks later, the killer began texting Georgie and she arranged to meet him in Rice Park. Tragically, she was later found strangled to death by Damian Spinelli, confirming the existence of a serial killer in Port Charles and leaving everyone in danger. Numerous individuals, including Sam McCall, Alexis Davis, and Maxie Jones, received threatening text messages, with Maxie and Alexis also being physically attacked, putting their lives in grave danger.
During the police investigation into her murder, DNA from Coop's skin was discovered beneath Georgie's fingernails. As the police were narrowing their focus on Coop, he stole the DNA results from the PCPD and requested that Maxie accompany him out of town, to which she consented. However, upon arriving at Coop's room at Kelly's, Maxie found him hanged. The PCPD classified it as a suicide, but Maxie remained unconvinced. She did not believe that Coop had taken his own life, nor that he was responsible for Georgie's death. Teaming up with Spinelli, Maxie set out to uncover the identity of the Text Message Killer, exonerate Cooper, and hold him accountable for the deaths of Leticia, Emily and Georgie. Among their primary suspects was Logan Hayes.

One evening at the docks, Sam received messages from the killer and was subsequently assaulted. After a confrontation, she escaped into the street, only to be struck by a vehicle. Following her recovery in the hospital, Sam returned to her apartment, where she encountered the Text Message Killer waiting for her. They engaged in another struggle, during which his identity was disclosed as Diego Alcazar.
Diego recounted the details of his survival to Sam. He explained how he had endured her shooting him, revealing that as his father, Lorenzo Alcazar, held his lifeless body, he detected a faint pulse and realized he was not dead. He then hired a doctor from Mercy Hospital to save him and accompany him to South America for his recovery. Diego mentioned that the last time he communicated with his father was on the night Jason Morgan murdered him. Lorenzo intended to travel to South America with Diego's sister Lila Rae Alcazar and her mother Skye Chandler-Quartermaine to visit him. Lorenzo had sent Diego a text message stating, "I am coming for you."
Sisters Maxie and Georgie Jones were both attacked by the killer. Maxie survived, Georgie did not.
Diego articulated how the death of his father transformed everything. He desired retribution for the wrongs inflicted upon his family: his father, uncle Luis Alcazar, and cousin Sage Alcazar. He wished for them to endure the same grief and suffering that he had faced. He murdered Emily solely because she was Jason's sister. He killed Georgie, who was tragically in the wrong place at the wrong time, as the message he sent had been intended for Maxie. He also killed Coop, mistakenly believing that Coop was close to uncovering his true identity. In reality, Coop suspected Diego's father, Lorenzo, rather than Diego himself.
As Diego spoke, Sam came to the realization that he intended to kill her. She attempted to resist him, but her injuries rendered her too weak. Diego abducted her and transported her to a warehouse. Subsequently, he visited Sam's apartment to stage it as if she had departed the area. As he was exiting, Elizabeth Webber was arriving. Elizabeth recognized the danger she was in and attempted to escape, but Diego apprehended her and kidnapped her as well. While Diego was placing an unconscious Elizabeth into his backseat, Max Giambetti arrived and understood the situation. The two engaged in a gunfight, but Diego managed to flee, running over Max in the process.
Elizabeth regained consciousness while Diego was driving. She tore off the ski mask he had been wearing and fought against him until he crashed the car, which came to a halt at the edge of a bridge. Lucky Spencer and Jason arrived to rescue Elizabeth and Sam, but Diego shot Lucky and struggled with Jason, inflicting injuries on both of his hands before escaping. As the car precariously balanced, Jason assisted Elizabeth in her escape, but was unable to save Sam as the vehicle plunged into the water. Nevertheless, Sam clung to the edge of the bridge and was rescued by Lucky, who subsequently received treatment for his injury.

Nikolas Cassadine ultimately recalled having seen Diego's face prior to the murder of Emily, and he along with his companion Nadine Crowell proceeded to the warehouse where it was believed that Sam had previously killed Diego. Nikolas had recollected who he had seen Emily with on the night of her demise. Following a dispute, Nikolas left in a huff, abandoning Nadine in the warehouse. Diego reappeared, abducted Nadine, and set up explosives within the warehouse. Upon his return, Nikolas was confronted by Diego, who launched an attack on him. He choked Nikolas and then departed. Nevertheless, Nikolas managed to survive and pursued Diego. He discovered Diego engaged in a struggle with Spinelli and Maxie. Nikolas then confronted Diego in combat, but during the altercation, a rope ensnared Diego's neck, leading to his fall from a platform. The Text Message Killer met his end by hanging.
