Background[]

The women in Jason's life watch as he is taken into surgery
After Jason is involved in a car accident with Carly Spencer, he suffers another brain injury and has to undergo surgery to remove a piece of dashboard that had been stuck in his brain since the 1995 car accident with his brother, A.J. Quartermaine, that left him with permanent amnesia. His life hangs in the balance as Dr. Patrick Drake performs a life-or-death procedure. No one is certain of who Jason will be when he wakes up. While Jason is in surgery, the women in his life (Monica Quartermaine, Elizabeth Webber, Carly Spencer, and Sam McCall) fantasize about how life could be different if Jason was who they wanted him to be.
Despite being featured prominently during the storyline, Robin Scorpio-Drake was the only one of Jason's past loves to not have an independent fantasy. Instead, she was seen as an observer of the surgery. This could be because the writers felt Robin already had closure from her relationship with Jason.
During the fantasies, each woman is seen as an observer in spirit form. They observe their own fantasies and also Jason's on-going surgery. At the end of each fantasy, the real Jason appears to them, also in spirit-like form, to share some truths about their relationship.
At the end of the episode, while still in surgery, Jason sees his life as Jason Morgan flash before his eyes and he recalls prominent moments in his life, including those spent with his family and past loves.

What If Jason Hadn't Been in A Car Accident

Dr. Jason Quartermaine, Chief of Staff
In Monica's fantasy, Dr. Jason Quartermaine is the new chief of staff at General Hospital and an accomplished cosmetic surgeon. Jason had not been in the car with A.J. when he crashed and sadly lost his own life. Alan enters the living room at the Quartermaine Mansion and expresses his pride in Jason's accomplishments and Jason and Monica embrace.
Edward enters to raise a toast to Jason, followed by Tracy. Tracy snarks at the family about how they probably bribed the board to get Jason the position, while Edward laments that Jason went into medicine instead of joining ELQ. Alan worries how Jason will juggle being chief of staff with operating his private practice. He assumes that Jason will give up his private practice, but Jason is confident that he can manage it all.

Stuart Damon returns as Dr. Alan Quartermaine
The family prepares to head to the hospital for a ceremony to honor Jason's promotion to chief of staff. Monica offers Jason a ride, but he declines in favor of taking the family Lamborghini. When Monica leaves, Jason takes out a packet of cocaine from his suit pocket. Monica in spirit form watches with concern as Jason snorts the coke, before dream Monica comes back and catches him. Monica confronts him, but Jason is unapologetic about his addiction, claiming it's the price to pay in order to be the Quartermaine's "perfect golden boy." Alan, Tracy and Edward return and want to know what the hold up is.
With Jason's drug addiction out in the open, Tracy makes more snarky comments and the family begins to argue. Jason storms out and Monica chases after him as he gets in his car and speeds off. The car crashes shortly after and Monica screams in horror. Later, the family gathers back together in the living room and discuss how the accident has left Jason in a coma with a cerebral edema and they don't know when he is going to wake up. Jason wasn't wearing a seatbelt and was thrown from the car upon impact. Monica laments that this was supposed to be the proudest night of Jason's life and instead it has ended in tragedy.
As the family continues to argue, real Jason appears to Monica's spirit also in spirit form to ask a question before walking away:
Jason: Did you want this to happen?
Monica: Of course I didn't.
Jason: Mom, you gotta let go and let me be who I am.

What If Jason Had A Different Profession

The Artist and the Architect
In Elizabeth's fantasy, she is a world-renowned artist and Jason is a successful architect. They are in a casual relationship, and enjoy traveling and seeing the world, but Elizabeth wants more. Elizabeth has just returned from an art show in Germany, while Jason is on his way to Tokyo for a job. Since both their jobs are so demanding, they initially agreed to keep things casual and not bring any expectations into the relationship. Jason enjoyed the freedom, but Elizabeth had other ideas.
After they reunite at her art studio, Jason finds an unopened pack of birth control pills. Elizabeth looks guilty when Jason accuses her of trying to get pregnant on purpose to trap him into a more serious relationship. Elizabeth doesn't see the harm in wanting more for them, but Jason disagrees. Elizabeth claims she didn't go off the pill deliberately, she just lost track. Jason reiterates that he wants their relationship to stay free and easy and becoming a father wasn't a part of that equation. In an attempt to make Elizabeth see things his way, he takes her to see his latest development project - an affordable housing community project.
Jason explains that his work is his baby and he enjoys being able to devote all of his time to his projects. Elizabeth argues that he could still be a father and devote himself to his work, but Jason disagrees, saying that it wouldn't be fair to a child. Jason doesn't want Elizabeth to have to give up anything just to be with him, so they both begin to see that they want different things from life and this would mean the end of their relationship. Elizabeth asked for a dance on the rooftop with Jason. Jason agrees and heads up the stairs to clear a tarp from the path. As he is heading up, he falls through the railing and crashes down unconscious onto the floor below.
Jason and Elizabeth return to her studio apartment and Jason is now in a wheelchair. Elizabeth does her best to take care of him, but Jason is bitter and upset about his new reality. He's upset that Elizabeth is stuck taking care of him, just like he is now stuck in the wheelchair and unable to do his job like he did before. Elizabeth tries to reassure him that she doesn't feel stuck, but instead Jason congratulates her on getting what she wanted. All he wanted was to be free, but now they were stuck together, just like she wanted. Elizabeth retorts that she did not want this and was just trying to be supportive, but Jason exclaims he would rather put a bullet in his head than live like he was living.
As Elizabeth's spirit looks on with dismay, real Jason appears, also in spirit form, and they exchange words before he walks away:
Elizabeth: I never really knew you, did I?
Jason: I'm not who you need.

What If Jason Gave Carly a Second Chance

The Mob Power Couple
In Carly's fantasy, she and Jason are in a romantic relationship. Jason took over the mob after Sonny had a nervous breakdown and left town. Carly took a leave of absence from the Metro Court Hotel to help Jason run the business, so they are essentially running it together as a mob power-couple. Jason and Carly are at Jake's playing pool and flirting. They start talking about the state of the business and Jason complains about how Sonny caused a lot of damage before he left town to which Carly remarks they are now left to clean up the mess.
Jason insists that he can run the business without Carly's help. But Carly proclaims she has waited so long to be with him again like this and now that they are partners in every sense, she is not going to give that up. Carly thinks that together, they are unstoppable. Jason gets a call and has to leave for an urgent meeting. Carly wants to go with him, but Jason refuses saying it's too dangerous. As they are about to kiss goodbye, Michael walks in and interrupts. He calls Jason "gangster garbage" and clearly disapproves of their relationship. Michael is still loyal to Sonny and feels that Jason and Carly betrayed him.
They argue about the events that led to Sonny's breakdown and Michael is bitter because he feels they didn't even try to help Sonny. Carly defends them and says what happened to Sonny was not their fault. Michael doesn't believe her and continues to rail on Jason and Carly before declaring that he hates Jason and the Quartermaines are now his family. The argument gets increasingly heated when Michael accuses Carly of abandoning him after he woke up from the coma different. Jason jumps to Carly's defense and Michael turns his anger onto him warning him that he's next. Carly is going to do to him what she did to A.J. and Sonny.
Michael storms out after saying they are now dead to him and he no longer wants to have any contact with them. Carly tells Jason to go after Michael, but as he does, they are stopped by the sound of gunshots. Carly runs out after Michael and Jason and finds Michael dead from a gunshot wound. Jason and Carly return to Jake's days later dressed in black after attending Michael's funeral. Carly begins to question all of her choices and Jason tries to comfort her. Jason says they have to end things between them or else she could die too and he couldn't live with that on his conscience. Carly tearfully disagrees recounting all of the ways that she needs Jason, but Jason is steadfast in his decision and leaves.
Carly's spirit watches in shock as the real Jason appears, also in spirit form, and speaks to her before walking away:
Jason: Why are you cutting yourself open over things that can never happen?
Carly: Never?
Jason: You don't want anything different from what we already have.
Carly: How do you know?
Jason: Because I know you.

What If Jason Moved to Hawaii with Sam

Mr. & Mrs. Morgan +1
In Sam's fantasy, she and Jason are married and living in Hawaii. After getting married and leaving for their honeymoon six months ago, they never went back to Port Charles. They are blissfully happy and spend their days snorkeling, surfing, swimming and scuba diving. Sam also went back to salvage work, diving for sunken treasure. They tease each other over the legend behind an ancient Tiki mask, when Sam suddenly recognizes the mask's design and realizes that it's a treasure map.
The mask could lead them to a sunken submarine from World War II the contains a boat load of gold. The map they have is not compatible with the mask, so they leave to find a new one. Jason and Sam return to their bungalow with a new map and Sam explains that the mask is like a key and if they position it the right way on the map, then they will find the location of the sunken sub. Jason is skeptical about the existence of the sub, but Sam presses on undeterred and determined to find it. As they continue locating the coordinates of the sub, a stranger with a gun appears outside.
They hear a noise and Jason grabs his gun before going out to investigate. He tells Sam to stay put while he goes after the intruder. Sam walks over to grab the mask and gets light headed, stumbling on her way back to the bed. The intruder comes in from a back room and demands that Sam hand over the mask. Jason comes in from the front door and demands that he drop the gun. The intruder takes Sam hostage and says all he wants is the mask. Jason tells Sam to give the mask to the intruder, but as she turns to do so, she passes out.
Jason punches the man and then throws him out, threatening to kill him if he sees him again. He then returns to Sam's side concerned. She claims she was faking to distract the intruder, but Jason isn't convinced and picks her up to take her to the hospital. When they return home, Sam says she is in perfect health but Jason is still concerned. She sits him down to explain why she passed out, but first she grabs her phone for a picture. She wants to take a photo to remember this moment for the rest of their lives. Jason curiously asks why and she tells him that she is pregnant as she snaps a photo of his reaction. They take another picture, their "first family photo" and kiss to celebrate their new addition.
Sam's spirit watches with joy and some uncertainty when the real Jason appears, also in spirit form. This is the only instance in the fantasy sequences where the spirits make physical contact and Jason does not walk away.
Sam: Are you sure this is what you want?
Jason: As long as I'm with you, I'm happy.
